- If you intend to submit for reimbursement, please please notify us prior to the start of your treatment.
- Contact Insurance company and determine if you have out-of-network benefits (or look online on your provider’s website).
- - BE SURE TO GIVE YOUR INFORMATION to get accurate information.
- Schedule Physical Therapy Movement and Gait Assessment at www.spineandmovement.janeapp.com and enter in your payment information.
- You will be given the detailed receipt and treatment note required for reimbursement at the time of visit (or emailed the same day).
- Submit the paperwork to your insurance company.
- Get reimbursed in 7-10 business days if you met your deductible.
