Insurance Out-of-Network
​Benefit Instructions

  1. Contact Insurance company and determine if you have out-of-network benefits (or look online on your provider’s website).
    ​- BE SURE TO GIVE YOUR INFORMATION to get accurate information.
  2. Schedule Physical Therapy Movement and Gait Assessment at www.spineandmovement.janeapp.com and enter in your payment information. 
  3. You will be given the detailed receipt and treatment note required for reimbursement at the time of visit (or emailed the same day). 
  4. Submit the paperwork to your insurance company.  
  5. Get reimbursed in 7-10 business days if you met your deductible.
